This Form is for (The DATE), 11:00 am Worship Service Only. Please fill out the form completely. All persons requesting attendance (including children that require a seat) Must be registered. Seating is limited for each service. Everyone one 2 years of age and older will be required to wear a mask for the entire service. Mask must be worn over both nose and mouth. Everyone’s temperature will be taken before you will be allowed to enter Worship service, and Washing Hands using sanitizer station is also required. Usher will seat all attendees according to CDC social distancing mandate,

As a result, Federal, state, and local Governments and agencies recommend social distancing and have, in many locations, prohibited the congregation of Groups of people. At Second Sweet Home Baptist Church (SSHBC), we are doing everything we can to be compliant with all regulations and ensure your safety. We have put in place preventative measures to help reduce the spread of COVID-19. But we cannot guarantee that you or your family members will not become infected with COVID-19.
